The cellar visits are in Vila Nova de Gaia, across the bridge, where the port houses have their lodges. You walk over, you are shown the barrels, the process is explained, you taste two or three wines, and you are back in Porto for lunch. From 25 EUR, and the whole thing is under two hours.
The Douro days leave the city between eight and nine in the morning and return in the early evening. Ninety minutes each way on the road, two estates, lunch, and on most of them an hour on the river. From 89 EUR, and it is the entire day.
Both are worth doing and they answer different questions. What they are not is substitutes: the cellars show you how port is aged and sold, the valley shows you where it grows, and the terraced schist hillsides above the river are the reason the landscape is a World Heritage site rather than just a nice drive.

What is actually in a Douro day
The itineraries converge, so it is worth knowing the components and which ones vary.
Two estates, usually. Sometimes one, occasionally three on the private days. A quinta visit means the vineyard terraces, the winery, often the granite lagares where grapes are still trodden by foot during the harvest, and a tasting of three or four wines.
Lunch. Included on nearly all of them, and it is a proper Portuguese meal rather than a sandwich. This is a large part of why the price sits where it does.
A river cruise. On most, and it is usually an hour on a modern boat rather than one of the old rabelo cargo boats you see moored at Gaia. Those are a photogenic survival: they carried the wine downriver before the road and the dams, and the ones on the water now are replicas or restorations.
The drive. Ninety minutes each way, and the last stretch along the river is the best part of it. Anyone prone to car sickness should say so: the valley roads wind.
What varies is the group size and whether the estates are large commercial ones or small family quintas. That is the difference between an 89 EUR seat and a 148 EUR one, and it is a real difference.
The harvest, and the treading
Picking in the Douro runs through September, and it is the one month when a visit shows you the thing itself rather than the equipment for it. Several quintas still tread part of their crop by foot in shallow granite tanks, which is not folklore for visitors: for the top wines it remains a working technique, because feet break the skins without crushing the seeds.
Some estates will let visitors join in during harvest, and a few listings advertise it. The trade-off is the same as everywhere: a working winery has less time for you, and some restrict visits precisely because they are busy.
Outside September the terraces are still spectacular, greener in spring and rust coloured in late autumn, and the cellars are quieter and better for actually talking to someone.

Three of these twenty are food tours filed under wine by their operators, and two more are city tours with a tasting attached rather than a cellar. They are honest about it in their own descriptions; the filing is just how the supplier categorised them.
It matters mainly for price comparison. If you are scanning this page for the cheapest wine tour and land on a 30 EUR seated tasting or a 49 EUR city tour, you will have booked something that visits no cellar at all. Is lunch included?
On nearly all the Douro days, and it is a full Portuguese lunch rather than a snack. That is a substantial part of what the 89 to 148 EUR covers. The cellar visits do not include food, but you are back in Porto in time to eat.
What is the river cruise like?
About an hour, usually on a modern covered boat, going upstream and back through one of the locks on some itineraries. The traditional rabelo boats moored at Gaia are restorations of the cargo craft that carried barrels downriver before the roads and the dams; they are photographed far more often than they are sailed.
Can I do the Douro by train instead?
Yes, and the line along the river from Porto is one of the most scenic in Europe. What the train will not do is take you to the estates, which sit on the hillsides above the valley floor and are not walkable from the stations. The train is a good day out; it is not a wine tour.
When is the harvest?
September. It is the best month to see a working quinta and the busiest, and some estates limit visits while picking. A few listings offer participation in the foot treading, which is a real technique rather than a show, and those sell out early.
Is 259 EUR for a private day worth it?
It is roughly three times the standard day. What it buys is a car rather than a coach, estates chosen to your taste, and the ability to stay longer where you want to.
